Treatment will start with a comprehensive psychiatric evaluation using a holistic approach to understand the many factors that contribute to mental health. We offer a calm, comfortable and confidential environment in which to explore your concerns. We strive to provide each client with a customized treatment plan that allows them to reach their goals and fulfill their potential. Our practice is designed to give you the time and individualized attention you deserve

Ages treated include adult: 18 years and above.

Special areas of interest include: mood and anxiety disorders, bipolar disorder, attention deficit, obsessive and compulsive disorder (OCD), insomnia, trauma disorders, women's mental health disorders and addiction and more.

⬥ Initial adult psychiatric evaluation

Comprehensive Psychiatric Assessment. Includes discussion of goals, identification of strengths and current challenges, diagnostic assessment and interview, education, and treatment planning.

60 min : cash pay OR go through insurance (We do provide a superbill that you can submit for reimbursement if you are out of network).

⬥ Medication Management Follow-Up (in person or telemedicine appointment)

For established patients, follow-up appointments will consist of brief psychotherapy, discussion of response to treatment, and reassessment of treatment plan as necessary.

30 min: cash pay or go through insurance (We do provide a superbill that you can submit for reimbursement if you are out of network).

⬥ Phone Consultation

For established patients only. For questions or concerns between scheduled appointments that do not require an in-person visit.

15 min: cash only or go through insurance insurance (We do provide a superbill that you can submit for reimbursement if you are out of network).

⬥ Paperwork:

For established patients only. We only offers filling out short-term disability form and FMLA paperwork. Appointment might be required to fill out the form to discuss details.

• Short-Term Disability / Accommodation form (school, work, government,etc)- $50.00/form

• Other forms- cost will be determined and provided to you prior to completion.
